Today, at approximately 9:15 a.m., the City of Fort Wayne Police Department responded to the intersection of Taylor Street and Fairfield Avenue reference a personal injury accident involving two vehicles.  First responders pronounced the driver of a 1999 silver-colored Chevy minivan deceased at the scene.  The passenger of that vehicle was transported to a local hospital where she remains in serious condition at this time.  The driver and sole occupant of the other involved vehicle, a 1994 Ford F250 pickup truck, was also transported to a local hospital in serious condition.

At this time, investigators of the F.A.C.T.
